Medical Release Form - A medical release form should be filled out and notarized for each player on the roster in case emergency medical attention is required and a guardian is not present to authorize medical services.

Summer Tournament Roster XLS - This blank roster can be used for summer tournament play. Each tournament has their own requirements for Rosters and check-in, please check their rules first! Larger tournaments have a roster form they require teams to fill out and have signed by the association or club registrar. At least 7 working days lead time is required.
